Model Participation &
Media Release Agreement

1. Model Program & Complimentary Services

Subtle Return agrees to provide the Model with the treatment sessions identified in the agreed Model Program schedule at no charge, representing a total retail value of up to $2,800.

In exchange for participation in the Model Program, the Model agrees to:

• Attend the scheduled consultation, treatment, touch-up, and healed-review appointments;
• Follow all written and verbal aftercare instructions throughout treatment and healing;
• Permit reasonable photography and video documentation of the treatment process and results as described in this Agreement;
• Participate in the scheduled healed-results photography and video session; and
• Communicate promptly with Subtle Return regarding healing concerns, scheduling problems, or circumstances that may affect participation.

The Model understands that no wages, cash payment, royalties, residuals, or other monetary compensation will be paid for the Model's time, participation, images, recordings, statements, or testimonials. Health and safety considerations will always take priority over completion of the Model Program.

2. Scheduled Model Program

Appointment dates may be modified by mutual written agreement or when reasonably necessary because of health, healing, practitioner availability, weather, emergency, or other circumstances beyond either party's reasonable control.

Subtle Return does not guarantee that the Model will require or receive a specific number of treatment sessions. The practitioner may modify, postpone, or discontinue treatment when additional pigment would be unsafe, unnecessary, or aesthetically inappropriate.

3. Attendance & Cancellation Requirements

The Model agrees to attend all scheduled appointments and arrive on time. The Model should provide at least 48 hours' notice when requesting to cancel or reschedule an appointment, except in the event of illness, emergency, unsafe travel conditions, or another circumstance reasonably beyond the Model's control.

Repeated missed appointments, substantial lateness, failure to communicate, or voluntary withdrawal without an excused reason may result in removal from the Model Program and cancellation of any remaining complimentary services.

Subtle Return may also reschedule an appointment when reasonably necessary for illness, emergency, practitioner availability, healing concerns, or safety considerations.

4. Aftercare & Healing Responsibilities

The Model agrees to follow all written and verbal aftercare instructions provided by Subtle Return throughout the treatment and healing process.

The Model understands that aftercare compliance is an essential condition of participation in the Model Program because failure to follow those instructions may affect healing, pigment retention, color, uniformity, and the appearance of the final result.

The Model agrees to notify Subtle Return promptly of any healing concern or circumstance that may affect the treatment result or the Model's ability to follow aftercare instructions.

Failure to substantially follow required aftercare may result in removal from the Model Program or cancellation of any remaining complimentary sessions.

The Model acknowledges that treatment outcomes vary between individuals and that participation in the Model Program does not guarantee a particular cosmetic result. Detailed treatment risks, contraindications, medical disclosures, and procedure-specific informed consent are addressed separately through Subtle Return's clinical intake and consent process.

5. Media Capture & Model Cooperation

The Model authorizes Subtle Return and its employees, contractors, photographers, videographers, representatives, successors, and assigns to photograph, film, record, and document the Model before, during, and after treatment.

Authorized media may include:

• Scalp and hair photographs and before-and-after comparisons;
• Video of the consultation or treatment process;
• Photographs or video showing the Model's face or other identifying features, if authorized by the Model;
• Audio recordings, written statements, recorded interviews, or testimonials; and
• Final healed-result photographs and video.

The Model agrees to reasonably cooperate with routine smartphone documentation during appointments and with the planned professional photography or video session at the healed-review appointment.

Subtle Return agrees to conduct media capture respectfully and to avoid unnecessary exposure of the Model's body or personal information.

6. Media Permissions & Authorized Uses

The Model grants Subtle Return a perpetual, worldwide, royalty-free right and license to use, reproduce, edit, crop, retouch, adapt, publish, display, distribute, advertise, and create derivative materials from the photographs, video, audio, statements, and testimonials authorized under this Agreement.

Authorized uses may include Subtle Return's website; social media; digital and print advertising; Meta, Instagram, Facebook, Google, YouTube, and similar platforms; email marketing; brochures and promotional materials; educational materials; professional portfolios; practitioner education and internal training; presentations; and other lawful business and promotional purposes related to Subtle Return and its services.

The Model understands that media used online may be viewed, copied, shared, reposted, archived, or distributed by third parties beyond Subtle Return's control.

7. Editing, Ownership & Future Media Requests

Subtle Return may edit, crop, resize, combine, add text to, or otherwise adapt authorized media for lawful business purposes. Subtle Return will not intentionally use the media in a false, defamatory, obscene, or materially misleading manner.

The Model waives the right to inspect or approve each final advertisement, design, caption, edit, layout, or publication before use, provided that the use remains within the permissions granted under this Agreement.

Subtle Return will not knowingly attribute a direct quotation or personal medical claim to the Model that the Model did not make.

Photographs, recordings, designs, edits, advertisements, and other creative materials produced by Subtle Return or its employees will be owned by Subtle Return. For materials created by an outside photographer, videographer, or contractor, Subtle Return's rights will be governed by its agreement with that creator. Nothing in this Agreement transfers ownership of the Model's identity itself.

The media rights granted under this Agreement are intended to be continuing and may be relied upon by Subtle Return when investing in treatment, photography, editing, advertising, and promotional materials.

The Model may submit a written request asking Subtle Return to discontinue new uses of identifiable media. Subtle Return will consider the request in good faith, but cannot guarantee removal of printed or previously distributed materials, completed or already-placed advertising, third-party reposts, search-engine archives, saved copies, or materials outside Subtle Return's control.

8. Withdrawal or Removal From the Model Program

The Model may withdraw from future participation at any time by notifying Subtle Return in writing.

Subtle Return may remove the Model from the program and cancel any remaining complimentary services if the Model repeatedly misses appointments, materially declines agreed documentation, substantially fails to follow required aftercare, or otherwise does not complete the agreed Model Program requirements.

The Model will not be charged for unperformed services or for treatment already completed under this Model Program.

A practitioner-directed pause or discontinuation for health, safety, candidacy, healing, or aesthetic reasons will not be treated as a failure by the Model.

9. Testimonials & Statements

The Model is not required to make a false, exaggerated, or scripted statement. Any testimonial provided by the Model must reflect the Model's honest experience and opinions.

Subtle Return may edit an authorized testimonial for length, grammar, clarity, formatting, or presentation, provided the edit does not materially change the meaning of the Model's statement.

10. Personal Information & Records

Subtle Return may maintain the Model's personal information, treatment documentation, photographs, consent documents, and related business records as reasonably necessary for legal compliance, business records, education, safety, and documentation.

Subtle Return will not intentionally publish the Model's phone number, email address, home address, date of birth, government identification, health-history form, or other private administrative information.

11. Governing Law & Entire Agreement

This Agreement will be governed by the laws of the Commonwealth of Virginia. Any provision found invalid or unenforceable will be interpreted or limited to the minimum extent necessary, and the remaining provisions will continue in effect.

Nothing in this Agreement is intended to waive any protection, remedy, responsibility, or legal right that cannot lawfully be waived.

This Agreement governs the Model's participation in the Model Program and the media permissions described here. Separate health-history screening, Virginia client disclosures, procedure-specific treatment consent, procedure records, and clinical aftercare documentation will be completed through Subtle Return's normal treatment process.

Any amendment to this Agreement should be in writing and accepted by both the Model and an authorized representative of Subtle Return.
